Why this is a doctor-led assessment
Ultimate is not a scan booked in isolation, because your GP takes your history, examines you and then reads the blood results, measurements and MRI report together. That context decides whether a finding needs action, monitoring, a further test or no treatment.
After the screening date you also have one year of follow-up GP access: four GP consultations, unlimited in duration, for you or your immediate family to use during the 12 months from screening. If a result later needs repeating or a plan needs adjusting, the same clinic can keep the thread.

Around 9%
of recent Ultimate MOT scans showed something that needed to be discussed at the results review. This does not mean that 9% had a serious problem, because some results may be harmless or need no treatment. Your doctor reviews the scan with your other results and explains whether anything needs to happen next.
Male and female options
All three options include the doctor examination, 100+ health markers, measurements and investigations, tumour markers (CEA, and PSA or the CA-125/HE4/ROMA panel), gluten sensitivity profiling, a full iron profile with B12 and folate, advanced thyroid and micronutrient testing, and full-body MRI of the brain, spine, chest, abdomen and pelvis.
Ultimate Male · £2,950
Full body MRI replaces standard X-ray and ultrasound. The male option also includes a full pelvic examination, male hormone mapping, CEA and, from age 40, PSA.
Ultimate Female with gynaecology · £3,350
Full-body MRI is additional to gender-specific ultrasound and specialist gynaecological review. This option adds a consultant gynaecologist consultation, pelvic and trans-vaginal ultrasound, cervical smear, mammogram, female hormone mapping, CEA and the HE4, CA-125 and ROMA ovarian panel.
Ultimate Female without gynaecology · £2,950
The same full-body MRI and expanded blood panel, without the gynaecology consultation, trans-vaginal ultrasound, cervical smear, mammogram, HE4, CA-125 or ROMA. Available Monday-Friday if you want the MRI-led assessment without the gynaecological component.
What the MRI looks at
- Brain and neck
- Chest, including lungs and major vessels
- Abdomen and pelvis, including liver, kidneys, pancreas, spleen, adrenals and pelvic organs
- Cervical, thoracic and lumbar spine
MRI uses magnetic fields and radio waves rather than ionising radiation. A consultant radiologist reviews the images. MRI can show structural change that blood tests cannot, but it is not a diagnosis, a broad cancer screen or a whole-body all-clear.
Blood tests unique to Ultimate
- Gluten sensitivity profile: tissue transglutaminase antibodies (tTG-IgA), total IgA and endomysial antibodies
- Full iron profile: serum iron, TIBC, ferritin and transferrin saturation, plus B12 and folate
A tumour marker result is a prompt for clinical review. It does not confirm or exclude cancer.

The Harley Street clinic assessment usually takes 2-3 hours. The full-body MRI takes 60-90 minutes at a partner imaging centre. Appointments run Monday-Friday.

Before
Continue regular medicines unless instructed otherwise, arrive hydrated, and fast only if Medical Express Clinic gives you a specific instruction for your selected tests. Fasting is not required before the MRI unless the clinic gives a separate instruction. Tell the team about implants, pacemakers, aneurysm clips, cochlear implants or metal in the eyes before the scan is booked.

At the clinic and MRI
Your GP takes your history and examines you, then the visit moves through vital signs, body composition, ECG, lung function, VO2 Max and blood collection. Women booking the gynaecology option have their specialist review and smear as part of the clinic visit; ultrasound and mammogram are arranged at a partner imaging centre nearby. The MRI takes 60-90 minutes at a partner imaging centre. You lie still in the scanner, with ear protection provided.

Results review
When the laboratory and imaging reports are complete, usually within 7-10 working days, you return so your doctor can go through the secure interactive digital report with you.
Results and follow-up
Your doctor reviews every result before the appointment, then explains the blood results and the MRI report together. Scans often show something unexpected, and many of those results are harmless or need no treatment. The review is there to put each result in context rather than leave you to interpret a scan alone.
If a finding needs treatment, repeat testing or specialist input, Medical Express Clinic can coordinate the next step through a Harley Street specialist network built over more than 40 years. The four included GP consultations give you one year of follow-up GP access from the screening date.
Ongoing care can stay within the same private GP practice rather than ending with a one-off screening report.
When Ultimate may not be the right fit
Ultimate suits adults who specifically want full-body MRI and the broadest investigation set in one doctor-led assessment. If you want a first baseline without imaging, start with the Essential MOT. The Advanced MOT adds respiratory and hormone assessment without MRI. The Elite MOT adds gender-specific ultrasound and specialist input short of full-body MRI. MRI and tumour markers can raise a question for further assessment. They cannot diagnose cancer, rule cancer in or out, or give a whole-body all-clear, and screening cannot rule out every future condition.
New, severe or worsening symptoms need a symptom-focused GP or urgent assessment rather than routine screening. If Ultimate identifies a concern, your doctor can arrange the focused test or referral appropriate to that finding.

Does the Ultimate MOT include a full-body MRI?
Full-body MRI of the brain, spine, chest, abdomen and pelvis is booked at a partner imaging centre. For men, MRI replaces standard X-ray and ultrasound. For women with gynaecology, MRI is additional to gender-specific ultrasound and specialist gynaecological review.

Do I need to fast?

Is the MRI scan suitable for everyone?
MRI uses magnetic fields and radio waves rather than ionising radiation. It is not suitable if you have certain metallic implants, such as some pacemakers, older aneurysm clips, cochlear implants or metal in the eyes. Modern joint replacements and dental work are usually acceptable. The imaging team screens you before the scan is confirmed.
What report will I receive after the Ultimate MOT?
You receive a secure interactive digital report of over 40 pages. Your doctor reviews every result before meeting you in person to go through the report, answer questions and agree any next step.
What happens if the MRI shows something unexpected?
Your doctor reviews the scan with your examination, medical history and blood results. Many unexpected scan results are harmless and need no treatment. Your doctor explains whether you need no action, monitoring, another test, treatment or specialist advice.
Can the Ultimate MOT diagnose cancer or give an all-clear?
MRI and tumour markers can highlight something that needs a closer look. They do not diagnose cancer, exclude cancer or provide a whole-body all-clear, and new or worsening symptoms still need a GP or urgent assessment.
