How to choose between an allergy consultation, skin prick test or ALEX3
Most patients visit us at one of two stages: either you are reacting to something and need to identify the trigger, or you already have a diagnosis and want consultant-led treatment.
1. Find the trigger
Choose the booking option that matches how specific your symptoms are and whether you can pause antihistamines:
- Unsure what is causing your reactions? Book the £250 consultation. Dr Pereira maps your symptom history and selects the right targeted blood panel or skin test during your visit.
- Suspect a specific trigger and want answers today? Book skin prick testing (£450). The fee includes Dr Pereira's consultation, up to eight allergens, and results in the room within 15 to 20 minutes.
- Have multiple possible triggers or cannot stop antihistamines? Book the ALEX3 blood test (£413.75) directly to screen 290+ allergens from a single blood sample, with results typically in five working days.
2. Start or continue treatment
Choose this path if you already have test results or a confirmed condition and need specialist management with Dr Pereira:

Why see a Consultant Immunologist (allergist) on Harley Street
Commercial postal allergy kits often return a confusing list of borderline positives without clinical context, leading people to cut out foods unnecessarily. Because a raised IgE level only shows sensitisation rather than a confirmed clinical allergy, Dr Pereira interprets every skin prick or blood result against your actual symptoms before recommending dietary exclusion or treatment.
When biologic treatment with Omlyclo (omalizumab) is needed, Dr Pereira gives each injection himself in clinic and reviews your response at every visit. Skin prick testing, ALEX3 and targeted allergy blood panels
We investigate suspected allergies using three complementary methods: in-clinic skin prick testing, molecular and extract blood screening with ALEX3, and targeted single-group IgE blood panels.
ALEX3 comprehensive allergy test
From one blood sample, ALEX3 screens sensitivity to 290+ allergens across whole extracts and molecular allergen components, covering foods (nuts, shellfish, milk, egg, wheat, fruit), tree and grass pollens, cat and dog dander, moulds, house dust mites and insect venoms.
It is particularly helpful when symptoms overlap or when you cannot stop taking antihistamines. Results typically arrive within five working days with a comprehensive written report (ALEX3 is not a same-day test).
Skin prick testing
Small drops of allergen extract are placed on your forearm and the surface of the skin is lightly pricked. When an immediate IgE sensitivity is present, a small raised wheal usually appears within 15 to 20 minutes, allowing Dr Pereira to read and explain your results during the same appointment.
The £450 fee includes Dr Pereira's consultation and up to eight allergens. Further allergens are £10 each, up to five more, and if you suspect a particular food or substance you may bring a sample to test in clinic.
Targeted blood panels
When your history already points to a specific group of triggers, such as nuts, shellfish, antibiotics, or aeroallergens, a targeted serum-specific IgE panel is often more focused than a 290-allergen screen. Any of the panels below can be taken during your consultation with Dr Pereira, with results typically back in two to five working days.

Food allergy testing
Immediate reactions after eating (such as hives, lip or eyelid swelling, wheezing, or vomiting triggered by nuts, shellfish, fish, milk, egg, wheat or stone fruit) are investigated with skin prick testing, ALEX3, or a targeted food panel. These tests measure IgE immune sensitivity; they do not diagnose lactose intolerance or non-immune digestive sensitivities.
Allergy testing for children
Dr Pereira sees children alongside a parent or legal guardian and advises whether in-clinic skin prick testing, the dedicated Children's Panel (£376.25), or ALEX3 is the most suitable approach for your child's age and symptom history.
Preparing for your allergy appointment and what to bring
Allergy consultations and skin prick testing run on Tuesdays from 14:00, with the last bookable window ending at 17:30. Appointments usually last 30 to 60 minutes.
What to bring

Before skin prick testing
Stop all antihistamines three days (72 hours) before skin prick testing so they do not suppress the skin reaction. Keep taking all other prescribed medicines as usual, including asthma inhalers and steroid nasal sprays. ALEX3 and targeted blood panels are not affected by antihistamines, so a blood test is the better choice if you cannot pause your tablets.

Interpreting your allergy results and starting treatment
While skin prick results are discussed in clinic within 15 to 20 minutes and blood tests return within two to five working days (typically five working days for ALEX3), a laboratory figure is only useful when matched to your clinical history.
Dr Pereira reviews your results against your symptoms to agree a clear management plan, whether that involves targeted trigger avoidance, rescue or daily medication, or allergen desensitisation (immunotherapy) when appropriate.
Omlyclo (omalizumab) at Medical Express Clinic
Omlyclo is a biosimilar of omalizumab (original brand Xolair), started and continued at Medical Express Clinic by Dr Scott Pereira only. Every dose is given in clinic by Dr Pereira as add-on therapy when licensed criteria are met:
- Severe persistent allergic asthma: add-on therapy to improve asthma control in patients with severe persistent allergic asthma.
- Chronic rhinosinusitis with nasal polyps: add-on with intranasal corticosteroids in adults aged 18 and above with severe disease when intranasal corticosteroids do not provide adequate control.
- Chronic spontaneous urticaria: add-on therapy in adults and adolescents aged 12 years and above with inadequate response to H1 antihistamine treatment.
Each visit is £500, which bundles your review consultation and the in-clinic injection. In Dr Pereira's practice at Medical Express Clinic, most patients on Omlyclo stay controlled for more than a month after each in-clinic dose, although this is observed clinic experience rather than a trial result and individual response varies.

Allergy vs food intolerance and when to seek urgent care
This clinic investigates suspected IgE-mediated allergies, interprets previous allergy reports, and provides consultant-led treatment including immunotherapy and Omlyclo where indicated.
Food allergy vs intolerance: Allergy testing looks for an IgE immune response. Digestive intolerances such as lactose intolerance work by non-immune mechanisms and require different investigations (note that our Gluten Sensitivity Profile is an immune sensitivity profile rather than a lactose or fermentable-carbohydrate intolerance test). If you are unsure which applies to your symptoms, a £250 consultation is the right place to start.
Severe or acute reactions: If a reaction involves difficulty breathing, wheezing, swelling of the tongue or throat, dizziness or collapse, call 999 or go straight to A&E. Medical Express Clinic is not an emergency service.
Mild seasonal symptoms: Mild hay fever or skin irritation that already settles with over-the-counter pharmacy treatment may not require a consultant immunologist; you can start with our GP services (£150, seven days a week).

How do I book allergy testing?

Can I book a test without a consultation?
You can book the ALEX3 290+ allergen blood test (£413.75) directly without a separate consultation. Skin prick testing (£450) already includes Dr Pereira's consultation in the fee. Targeted single-group IgE blood panels are selected and taken during a £250 consultation.
Does a positive test mean I have an allergy?
Not necessarily on its own. A raised skin prick or blood IgE result shows immunological sensitisation rather than a standalone diagnosis of clinical allergy. Dr Pereira interprets every positive finding against your actual symptoms and reaction history before recommending avoidance or treatment.
How soon will I get results?
Skin prick test results are read and discussed during your appointment, usually within 15 to 20 minutes. ALEX3 blood results typically take five working days, and targeted IgE blood panels typically take two to five working days.
Do I need to stop antihistamines before skin prick testing?
Stop all antihistamines three days (72 hours) before skin prick testing so they do not mask the skin response. Continue all other prescribed medicines as usual, including asthma inhalers and nasal sprays. ALEX3 and targeted blood panels do not require stopping antihistamines, so let us know when booking if you cannot pause your tablets.
